Office Visit We realize patients may need to change their appointments, however, we request a 24 hour notification of cancellation. That way the office has ample time to offer the opening to another patient. If you fail to cancel your appointment with your provider without due notice, we reserve the right to charge you a cancellation fee of $25.00. This fee will not be submitted to your insurance company, but will be your responsibility to pay.Diagnostic Testing You must be on time for any diagnostic testing scheduled (vascular testing, ultrasound, nuclear stress, and sleep study). Cancellation of a Nuclear Stress test or Sleep Study must be cancelled 24 hours prior to your scheduled appointment time to all Mapleview Medical staff plenty of time to schedule someone else. Same day cancellation for these tests will result in a fee of $200.00. This fee will not be submitted to your insurance company, but will be your responsibility to pay. |
